Existing Patient Appointments

If you need to schedule a follow-up visit and did not schedule an appointment at your last office visit, please call (512) 458-6656 ext. 2 or email us. Our office will provide a courtesy call two days in advance to notify you about the appointment. Please read our no show / cancellation policy if you do not call us with 24 hours of your appointment.

You will be asked to show your insurance card at every visit. This ensures that we bill the proper insurance company. Please arrive early to complete the Return Patient Form or complete it and bring it with you.

If you have seen other doctors for your current diagnosis or have had labs drawn recently, please have those faxed to the office or bring them with you. This will help the physician with your treatment plan.

Before your appointment, please notify our staff if there are any changes to your insurance or demographic information. All balances including copays, deductible, and co-insurances will be due at time of service.
