New Patient Appointments

New patients who have been referred from another physician for a consult or diagnostic test need to complete either the EMG/EEG Test or Consult Referral Form online first. Completing these forms online will expedite a quicker response from the clinic. You may also contact the new patient coordinator at (512) 458-6656 ext. 3. If you have not been referred to Dr. Westgate from a physician, please visit your primary care doctor first to be evaluated to determine if you need to see a neurologist.

Once you have been scheduled to see Dr .Westgate, you will receive an email confirming the date and time of your appointment. The email will also contain all of the new patient paperwork that needs to be completed and returned to our office before your appointment. New patient paperwork is due back to the office within 48 hours after receiving the email. We ask for this paperwork back so quickly since Dr. Westgate takes an extensive history of your medical condition. She will review your past medical history, previous labs, progress notes, tests, diagnostic reports, and studies before your appointment. Please note that if these materials are not present before your consult, your office visit may be postponed until all of the requested information is on site.

At your first visit, please arrive 15 minutes early to ensure all information is complete. For testing patients only, please arrive 30 minutes early if you have note completed and/or returned any forms to the office. This will ensure that your appointment is not delayed and Dr. Westgate is able to remain on time for other patients.

You will be asked to provide a valid photo ID, insurance card(s), email address, and telephone number(s) to the front desk. Your insurance benefits will be verified before your visit and you will be notified of your financial responsibility before you see the physician. All appropriate deductibles, co-payments and out-of-pocket expenses are due at time of service. Austin NeuroCare does not bill patients for their financial responsibilities. Patient portions can be estimated and will need to be paid at the completion of your visit. We accept cash, checks, money orders, Visa, Master Card, Discover and American Express.

If your insurance plan requires a pre-authorization or referral, you must provide Austin NeuroCare with a current referral or you will be responsible for payment of services provided. It is recommended that you bring a copy of your referral/authorization to your appointment.

If your plan requires you to have a primary care physician, it is your responsibility to make sure your insurance company has his/her name on file.
