Scheduling An Appointment
- Please call 512-458-6656, ext. 12 or send us an email.
- Please include the following information if you leave a voice
mail or send an email:
- First and last name of patient
- Type of appointment requested
- New Patient
- Existing Patient
- Previous Dr. Juren patient
- Existing and former Dr. Juren patients please include your
date of birth
- Please leave the best number along with an alternate number
and the best time to contact you
- If you have not received a return call within 24 hours, please
call us again
- Please have your insurance card available
- If your insurance requires a referral, please contact your primary
care physician to request the referral in advance
- Medical records from referring and/or primary care physician,
labs, x-rays, MRI's or other imaging studies can be sent to our
office prior to your scheduled appointment at 512-458-1035
Cancellations
- We confirm our appointments one week in advance
- Please call us to confirm receipt of our message if we do not
speak to you directly
- We realize that at times, appointments must be canceled, we ask
that your please give our office at least 24 hours notice if you
need to cancel your appointment
- This courtesy will make it possible to give your appointment to
another patient
- Appointments that are not cancelled within the 24 hour time frame
will be charged a $25.00 fee
Appointment Day
- Please print and complete our patient information
forms
- If you cannot complete the forms prior to your visit, please
arrive 15 minutes prior to your scheduled appointment
- Please bring your insurance cards and drivers license
- Co-pays and applicable deductibles will be collected at time of
service
- We accept cash, checks and most major credit cards
- Please bring a list of all current medications
- Please bring all records, labs, films, etc if they have not been
faxed to our office prior to your visit
Payment Policy
Austin NeuroCare is a provider for Medicare and most major insurance
plans. We provide insurance billing. Anything not covered by insurance
will be your responsibility.
We request payment at the time of your appointment for services that
are not covered by insurance. Your insurance company may also require
you to pay a co-payment at the time of your appointment. When necessary,
our staff will work closely with patients who require a payment plan.
If you have any questions regarding which insurance plans we accept
or any patient billing concerns, please call us at the phone number
below. Questions regarding your coverage and benefits should be directed
to your employer or insurance company. Thank you in advance for your
cooperation in helping us serve you better. We look forward to meeting
you soon.
Pharmacy Refills
- Please have your pharmacy fax us your prescription refill request
- Please allow 48 hours for your prescription to be filled
