New Patient Appointments

Doctor's Desk

New patients who have been referred from another physician for a consult should complete the Consult Referral Form online, or may contact the new patient coordinator at (512) 458-6656 ext. 3. If you have not been referred to Dr. Westgate from a physician, please visit your primary care doctor first to be evaluated to determine if you need to see a neurologist.

Once you have been scheduled to see Dr. Westgate, you will receive the new patient paperwork electronically, which needs to be completed and submitted prior to your appointment.. We ask for this paperwork back quickly, since Dr. Westgate takes an extensive history of your medical condition. She will review your past medical history, previous labs, progress notes, tests, diagnostic reports, and studies before your appointment. In order for you to keep your appointment, we must receive all completed new patient paperwork and records, at least 7 days prior to the appointment date.

Your insurance benefits will be verified prior to your appointment and you will be notified of your financial responsibility before you see the physician. All appropriate deductibles, co-payments and out-of-pocket expenses are due before the time of service. You will receive a call from our office to collect payment. Patients do have the option to keep a credit card on file for all future payments.  Austin NeuroCare does not bill patients for their financial responsibilities. We accept cash, checks, money orders, Visa, Master Card, Discover and American Express.

Doctor Taking Notes

If your insurance plan requires a pre-authorization or referral, you must provide Austin NeuroCare with a current referral or you will be responsible for payment of services provided. It is recommended that you ensure we have a current copy of your referral/authorization form on file prior to your appointment. If your plan requires you to have a primary care physician, it is your responsibility to make sure your insurance company has his/her name on file.

A Return Visit Form will be sent to you to complete electronically 48 hours prior to your appointment. You must complete this form and verify all medications in order for you to be seen by a provider. If you have seen other doctors for your current diagnosis, or have had labs drawn recently, please have those records faxed to the office. This will help the physician with your treatment plan.

Credit Card Purchase

Before your appointment, please notify our staff if there are any changes to your insurance or demographic information. All balances including copays, deductible, and co-insurances will be due before the time of service. You will receive a call from our office to collect payment. Patients do have the option to keep a credit card on file for all future payments.
